The Conversation Project is an on-line program, working in collaboration with the not-for-profit Institute for Healthcare Improvement (IHI), dedicated to helping people talk about end-of-life care. Hospice & palliative care nurses and ‘Conversation Project’ volunteers Arlene Lowney and Arza Goldstein will lead you through a “Starter Kit,” where you will answer questions and learn about resources to help with these conversations, including how to choose and be a Health Care Proxy.
